On December 18, 2003 at about 12:26 central standard time, Fedex Flight 647, a Mocdonnell Douglas DC-10 flying from Oakland International Airport to Memphis, Tennessee was landing on runway 36R in Memphis, and almost immediately the right landing gear collapsed. The plane veered off the right side of the runway, catching fire as it did so. The co-pilot received minor injuries as she evacuated the aircraft, as did one of the five non-revenue FedEx pilots who were on board as passengers. It was later discovered that the non-revenue pilot who activated the slide for the emergency exit had not been adequately trained in its operation, and so accidentally pulled the handle that released the slide as a raft in the event of a landing on water, so that the slide detached from the airplane.Several of the pilots who were on board as passengers stayed back to retrieve personal items from the burning aircraft, thus unnecessarily endangering themselves.Thankfully, all onboard survived. An inestigation revealed that although the aircraft had encountered a crosswind during landing, the conditions were well within the safe capabilities of the aircraft. However, it was discovered that the first officer did not properly line up the plane before touchdown, nor did she slow the plane adequately before touchdown, so that the plane came down excessively hard. As the plane came down, due to the crosswind, the right wing suddenly lowered approximately six degrees. This was beyond the design capabilities for the right main landing gear, and it snapped as a result. Investigators also cited the captain for failing to check the work of the first officer.Onboard this flight were 5 passengers and 2 crew members. Both crew members suffered injuries but all 7 people onboard survived. NOTE: The accident occurred on the last day of the flight crew's 4-day, 3-leg trip during which the captain of the flight, a company check airman, was conducting a line check for the first officer. The first leg of the trip departed MEM about 2100 on December 15, 2003, and arrived at Indianapolis International Airport (IND), Indianapolis, Indiana, about 2215 (2315 eastern standard time). The captain was the flying pilot for that leg. The second leg of the trip departed IND about 1431 (1531 eastern standard time) on December 16 and arrived at OAK about 1850 (1650 Pacific standard time). The captain and first officer remained in Oakland on December 17, with no flight duties assigned, then reported for duty on December 18 before the accident flight's scheduled departure time of 0810 (0610 Pacific standard time). The airplane's departure to MEM was delayed until 0832 because of a package sorting issue that was later resolved. The first officer was the flying pilot, and the captain performed the nonflying pilot and line check airman duties. Simple mistakes...led to an extreme crash landing caught on tape
